Qualified Social Worker

We have a fantastic opportunity for a GSCC registered Social Worker in an Adult mental Health in the South West London area.

Responsibilities:
1. Work in partnership with other professionals and agencies to undertake a holistic assessment of vulnerable adults and their families and carers within the legislative framework and using the eligibility criteria for services.
2. Develop care plans, which will be informed by an assessment of need and risk and identify desired outcomes within clearly defined timescales.
3. In developing care plans consider least restrictive options that encourage the service user’s independence and choice and control over their lives.
4. Ensure that service users’ views, and/or families/carers views are fully recorded during the assessment process and wherever possible are reflected in the subsequent case planning.
5. Negotiate with service users/families/carers to enable needs to be positively met within available resources.
6. Prepare and present reports in a clear and logical way, and contribute to multi-agency/multi disciplinary conferences and meetings as appropriate.
7. Ensure that follow up action from the conferences/meetings is undertaken.
8. Include cost/’best value considerations in identification of resources and case planning.
9. To undertake a comprehensive review of the assessment.
10. To undertake assessments as a member of the multi-disciplinary team and the duties of care co-ordinator under CPA.

Pay rate is between £23 and £28 per hour.

GSCC Registration is essential.
